How to Apply for a Birth Certificate

Understand the Registration Window

A birth must be registered within 21 days of occurrence. Hospitals and nursing homes automatically report births. For home births, the head of household must notify the local registrar (municipal corporation / gram panchayat) within 21 days.

Collect Required Documents

You will need: (a) Hospital discharge summary or birth proof letter signed by doctor/midwife; (b) Parents' Aadhaar cards; (c) Parents' marriage certificate; (d) Proof of address; (e) Affidavit if registering late (after 21 days). For delayed registration beyond 1 year, a magistrate's order is required.

Apply Online via State Portal

Visit the Civil Registration System portal (crsorgi.gov.in) or your state's municipal corporation website. Register/login, select "Birth Registration", fill in the form with child's name, date/time/place of birth, and parents' details. Upload scanned documents and submit.

Visit Local Registrar Office (Offline)

If applying offline, visit the local municipal office or gram panchayat. Collect Form 1 (birth registration form), fill it out, attach documents, and submit to the registrar. Pay the prescribed fee (usually ₹10–₹50 depending on state). For late registration, additional fees and affidavit apply.

Collect the Birth Certificate

After verification (usually 7–15 working days), the birth certificate is issued. Online applicants can download a digitally signed PDF. For offline applicants, collect the physical certificate from the registrar's office. The certificate is a permanent legal document — store multiple copies safely.

What if I don't have a hospital birth proof?

For home births or very old unregistered births, you can submit an affidavit from two witnesses (neighbours/relatives) attesting the birth. In some states, a certificate from an Anganwadi worker or school admission records are also accepted as supplementary proof.

Can I add the child's name later on the birth certificate?

Yes. If the child's name was not included at the time of registration (common in India), you can apply for a name inclusion within 15 years of birth at the registrar's office by submitting an application and a small fee.

Is a birth certificate mandatory for school admission?

Yes. Under RTE Act, a birth certificate is required for school admission. It is also mandatory for passport, Aadhaar enrollment for minors, voter ID, and most government benefits.
